Polo shrunk

I bought this polo a month ago and got it embroidered with my company logo. Unfortunately it shrunk from a medium to a small/xs and the collar shriveled up after one wash on cold so I will not be able to wear it. I have 15 or so other Peter Millar tops and this is the first time this has happened.
